To join by check, click on the membership application link located in the green column to the right on this page. Then, e-mail the application to executivedirector@uacdl.org or send the form by U.S. mail to UACDL, P.O. Box 510846, Salt Lake City, Utah, 84151. Mail the check to the same address.
